It is possible to add fields specified for the user in "Malfunction/Breakdown" folder of the PM Notification?

What additional fields do you want Fox? Is it for long text for users to enter? If yes, I thought users can define the malfunction/breakdown better in the item record with object part, damage & cause codes.
