Has anyone done a Risk Assesment on Oil Sampling procedure?

We don't have a formal risk assessment, but we have sampling procedures specific to the equipment that include precautions

Some example precautions:
1 Don't sample machines with small reservoirs from a drain while running – too much risk of losing oil level.
2 If sampling using a vacuum oil sample pump from the top of reservoir, stay clear of any rotating parts.
3 Close all opened valves and restore any removed plugs upon completion of sampling.
4 If air is sucked in rather than oil coming out during sampling, immediately close the valve (particularly for transformers).
5 Sampling of energized instrument transformers is not allowed.
