How about www.desmaint.com and their MAINTelligence package. They used to offer just the MAINTelligence Monitor software that tracked and trended all the data in the one package, but now most software packages can do that now.

SKF's Machine Analyst is another option. SKF more recently just came out with the Lubrication functionality built into their software.
