Dr. Penrose,
I talked with Keith and the process was actually developed by the auto and I think possibly the aircraft industry as well.

It takes into account Severity, Occurance and Detection whose output is of course a percentage.

Keith built the spreadsheet for our purposes and is company sensitive.

Dear Josh,

FMEA is used in RCM analysis and forms the first four steps. Bob Latino's company uses a modified FMEA to do RCAs. FMEA and FMECA are used in design studies to improve product or process reliability. So, as you can see, FMEA is a well established tool in Reliability improvements.
