I thought this statement says it all.

"A Proactive Asset Reliability Process is a supply chain. If a step in the process is skipped, or performed at a substandard level, the process creates defects known as failures. The output of a healthy reliability process is optimal asset reliability at optimal cost."

Look like your KPIs focus on the maint work process only but your theory mentions design & operating practices. How do you measure design & operating practices?

I tot I have posted something similar earlier which mean I wasn't offtrack that far.

Nice definition; it also ties in with the Goldratt approach. Sorry i missed you at RCM 2006.
