There are a number of work processes which need to be reliable and repeatable in the first place in order to produce excellent results when followed and applied such as :
1) RCA/FTA, FMEA/RCM, RBI, IPF, etc
2) work selection based on risk matrix, work planning and scheduling, work execution, etc
3) identification of bad actors, lost production opportunities, improvement intiatives, etc
4) identification and tracking of leading and lagging KPIs along with strategies for their improvement
5) planning and implementing an overall roadmap for plant performance

How to ensure the reliability of the above work processes in order to be consistent in our quest for permanent defect elimination?
