Does anyone use a formal request form for plant customers to request reliability services? If so, could a copy be shared? We are trying to formalize the requests for assistance from our department to segregate casual maintenance engineering requests from Reliability analysis projects.

Isn't a phone call and possibly followed by an email sufficient? And Reliability dept keep track those requests using a list.

I use PDMe(e=emergency)as a code in the CMMS and have added other filter codes to specify which tool was called for such as, ITA for Infrared Thermal Analysis, VIB for Vibration analysis and so on. Works very well if you have a CMMS.

Thanks, the notes, e-mails, napkins, phone calls, "hey yous" etc. all expect equal billing. The use of SAP (our CMMS) could facilitate the R/E work the same as other requested work. We have a work center and planning group for Reliability and could easily manage, plan, backlog, schedule, and communicate our intensions, progress, costs, and progress in the same system as maintenance work. BRILLANT!
