Do you have mean time between failures for all of these? (or is this what your RPN value is? If not, what is RPN?)

What is the cost for each of these failure modes? Is this weighted into your "severity" index? Also, what is the cost of each "recommended action"?

My first thought is that the maintenance procedures are not clearly defined. There seem to be several of these that are due to incorrect maintenance. If the procedures are clear and correct, I would recommend a training plan and a "quality assurance" inspection of a percentage of maintenance performed on this equipment.
